Comunicare aims to support patients in the context of chronic diseases by providing organizational support. It provides personalized information on the patient care plan, appointments and recommendations for a better quality of life. The patient can share most of this
information with his entourage. In return, the patient is asked to regularly share his outcomes and vital signs with the medical team for a better evaluation of the therapy.
